Partnership for a Healthy Community, Inc., was formed in 1994 and was as one of the first collaborative organizations in Florida to address community health. Originally, our mission was limited to conducting health needs assessments of Escambia and Santa Rosa Counties and supporting the efforts of community healthcare organizations. For the first twenty years of our existence, the Partnership conducted four comprehensive health status assessments (1995, 2000, 2005 and 2012). The Partnership also sponsored community forums, assisted area health organizations obtain grant funds and supported a variety of initiatives to improve health status for area residents.
Beginning in 2014, the Partnership expanded our role to become a collective impact backbone organization. As a backbone organization, Live Well Partnership seeks to bring community organizations together to achieve sustainable improvement to the health and quality of life of all people living in Escambia County and Santa Rosa County. In 2014, the Partnership published the Roadmap to Wellness, which identified collaborative actions to reduce high tobacco use rates, poor access to care, and high rates of obesity.
In 2016, the Partnership changed our name to Live Well Partnership for a Healthy Community to more accurately reflect our emphasis on helping our community live healthier lives.
