Community Health Needs Assessment (CHNA) Preliminary report

Every three years, Live Well Partnership conducts an assessment of the health of Escambia County and Santa Rosa County residents.  The purpose of the assessment is to identify the causes of poor health within our community and to stimulate community action that will make Escambia and Santa Rosa healthier places to live, learn, work and play.

This document presents the preliminary findings from this year-long process.  This report was presented to the CHNA Steering Committee, comprised of over 30 local organizations, on August 21, 2018.  Based on the findings within this report, the Steering Committee identified the top 4 priority health issues within each county.

CHNA Steering Committee

In March 2018, Live Well Partnership brought together a group of interested organizations to help us conduct the Community Health Needs Assessment.  The CHNA Steering Committee, as this group was named, is comprised of representatives from 34 health care providers, businesses, service organizations, governmental bodies and faith-based organizations.  

The CHNA Steering Committee has provided insight and guidance during the data collection phase of the assessment.  And, its members have actively participated in identifying community forces that impact health and healthcare delivery, listing resources available with the community and prioritizing healthcare issues.
